Make a Claim
  1. Download and print out the .PDF of the claim form. Download Claim Form Here

  2. Fill out the top portion (Part 1) of the form. Please make sure you provide all required information and print clearly.

  3. Bring the claim form to your veterinarian to fill in Part 2 of the form and sign.
    Note: Ask your veterinarian if they are using EVE (Online insurance claims filing for veterinary hospitals and clinics.)

  4. Sign and date your claim form before you fax, mail or email it in.

I'm writing to express my heartfelt gratitude in regards to the assistance PetCare provided me when my husky, Skye, became suddenly ill. A large bump was discovered in her chest on a Thursday. I scheduled an appointment with her vet for Friday... Unexpectedly, I had to leave the office without my angel because the vet informed me that she had a tumor that required immediate removal.
